Use of constellations in different areas: #3 Healthcare

We continue with a series of articles presenting interesting and reliable case studies on the use of constellations in different sectors around the world. Both research articles and documented experiences from practitioners are used as sources.

In the previous parts of this series, we looked at how the constellations method has been applied to. in the legal system and business and governance. This time, we delve into mental health and healthcare.

The constellation method in mental health and health care

Family constellations have been widely used in psychotherapy and personal development. It aims to support people by helping them to become aware of intergenerational emotional patterns and to bring symbolic relief to chronic problems such as depression, addiction or relationship conflicts.

Over the past decade, significant steps have been taken to gather scientific data on the effectiveness of constellation therapy. 2021. A systematic review and meta-analysis of 12 quantitative studies with a total of 568 participants was published in 2008 and gave a moderately positive assessment of the effect of family constellations [1]. On the Belong blog we have reported on this study in more detail. One randomised control trial found that participants in the family constellations group improved their psychological functioning significantly more than those in the control group, indicating a specific therapeutic effect of the method.

Around three-quarters of the studies analysed showed a marked improvement in clients’ well-being after participation in the constellations – measured by a reduction in psychopathological symptoms, an increase in self-esteem and an improvement in family relationships. One randomised control trial found that participants in the family constellations group improved their psychological well-being significantly more than those in the control group, indicating a specific therapeutic effect of the method [1].

At the same time, the scientific literature also stresses the need for caution, as the volume and methodological quality of existing studies is currently still uneven. However, the overall conclusion is promising: “the accumulated evidence points to the efficacy of family constellation therapy and its significant benefits in supporting mental health in the general population” [1].

Case studies and therapeutic experiences

In therapeutic practice, there have been several cases where the constellation method has brought about positive changes. For example, a story of a patient with depression who improved significantly after five family constellation sessions is described – a qualitative analysis revealed that the relief was linked to her new understanding of and acceptance of ancestral trauma PDF [1].

There have also been studies with patients with addiction problems where awareness of family secrets and repressed emotions through constellations has contributed to treatment outcomes.

It is important to note that the process of family constellations can be emotionally intense and may not suit everyone. In some studies, a small proportion of participants (around 5-8%) have reported short-term negative effects, such as increased anxiety or strained family relationships immediately after the session [1]. Usually these effects are transient and professionals stress the importance of aftercare to support participants’ further inner work.

In general, there is a cautious but positive attitude towards family constellations in the health sector – the method is not considered a classical science-based treatment, but it is seen as a complementary intervention that can provide valuable additional support in certain cases.

Formal recognition of family constellations and international initiatives

As a token of appreciation, family constellations were included in the national programme of integrative public health in Brazil in 2018. The Brazilian Ministry of Health treats it as a form of alternative therapy, similar to acupuncture and herbal medicine, allowing people to access them in public health centres to support holistic wellness.

At the same time, the decision sparked a lively public debate, with supporters stressing the need for better access to a popular form of therapy, and critics pointing to the need for more evidence. [2]

There is also a growing interest in family constellations in European healthcare. For example, in Spain (Autonomous Community of the Basque Country), a government agency recently funded a training course on “Family constellations: deepening the systemic view”, aimed at community and social work practitioners [3].

The aim of the 50-hour course is to teach awareness of the deeper level of family patterns and conflicts and to use constellation work to support the balance of different systems: family, education, work and society. It is a good example of how alternative therapy has entered the public sector training agenda. [3]

Summary of the article series

The Constellations method has been applied in a wide variety of contexts, from courtrooms and corporate boardrooms to hospital therapy rooms and public training sessions. Each case presented shows that a systemic view can reveal the hidden patterns and connections that shape our lives.

Scientific studies point to possible positive effects, although they stress the need for further research. Practitioners’ experiences, however, tell vivid stories of families finding reconciliation, teams regaining unity and individuals achieving personal breakthroughs.

Each of the cases presented here – be it the Argentine court’s innovation in justice [4], the widespread adoption of the Brazilian justice system [2], the healing of hidden dynamics in organizations [5], or the improvement of mental health in groups [1] – offers a fragmentary insight into how a systems perspective on family constellations can offer novel solutions to human problems.

A common denominator unites all these examples: the desire to look at the larger system beneath the surface and bring to light the connections and patterns that invisibly affect us.
